Please read carefully the following before signing this form. By signing, you accept all the below conditions:

1. Applications from Edison-NJ and the immediate vicinity will be processed at Care & Share Foundation.
2. Requested assistance may be in the form of checks, food certificates, or referrals.
3. Care & Share Foundation has permission to verify all provided information and share it with other Masajid or agencies if required.
4. Applicant agrees that assistance may be reported to state or federal agencies.
5. The Foundation does not guarantee the confidential security of personal information.
6. False information will lead to immediate disqualification.
7. Recommendation letter from your local Masjid is required.
8. All supporting documents (bills, IRS filings, ID) must be attached.
9. Applications are reviewed within 15 days; we do not provide urgent/immediate assistance.
10. The decision of the Zakat Committee is final and will not be provided in writing.
11. Applicant confirms they are not involved in any criminal activities affecting the safety of the community.
12. Applicant acknowledges they stand before Allah (SWT) in truth.
