Please complete this form if you wish to receive Eid assistance. By signing, you accept all the below conditions:

1. This form is intended exclusively for individuals who are Zakat eligible and have SNAP benefits.
2. To receive assistance, you must present your physical SNAP Benefits Card and a valid State ID. Photos or digital copies on a phone will not be accepted.
3. The applicant's name on this form must exactly match the name on the submitted ID.
4. Only one application is allowed per household. Multiple applications from the same household, including by different family members, will be reviewed and may all be denied.
5. All assistance is provided on a first-come, first-served basis, while supplies last. No specific item, type, size, or quantity is guaranteed, and items may be substituted based on availability.
6. Only the applicant named on this form may pick up, and your ID will be checked at pickup.
7. If approved, you must confirm whether you will attend at least 5 days before the event date. If you do not confirm, your spot may be offered to another family.
8. Pickup address and time will be shared after you register.
9. We reserve the right to deny assistance at our discretion.
10. Care & Share Foundation has permission to verify all information provided in this application.
11. False information will lead to immediate disqualification.
12. We will communicate with you by email only. Please make sure the email address you provide is correct and check it regularly, including your spam/junk folder, for your pickup information if approved.
