Important Information

Please complete this form if you wish to receive Food Assistance. By signing, you accept all the below conditions:

1. This form is intended exclusively for individuals who are Zakat eligible and have SNAP benefits.
2. To receive assistance, you must present your physical SNAP Benefits Card and a valid State ID. Photos or digital copies on a phone will not be accepted.
3. The applicant's name on this form must exactly match the name on the submitted ID.
4. Your government-issued ID must be currently valid (not expired) at the time of application.
5. Assistance will be provided while supplies last.
6. We reserve the right to deny assistance at our discretion.
7. Pickup address and time will be shared after you register.
8. Care & Share Foundation has permission to verify all provided information.
9. False information will lead to immediate disqualification.
10. We will communicate with you by email only. Please make sure the email address you provide is correct and check it regularly, including your spam/junk folder, for your pickup information if approved.
